When Questions About Sexual Orientation Become an OCD Cycle
Sexual Orientation OCD, often shortened to SO-OCD, is a commonly used term for an OCD presentation in which intrusive doubts and compulsive behaviors center on sexual orientation. People of different sexual orientations can experience this OCD theme.
It is important to approach this area carefully and without assumptions. Questioning or exploring one's sexual orientation is not a disorder, and therapy should never attempt to determine, change, or prescribe a person's sexual orientation.
When the Search for Certainty Becomes the Problem
With SO-OCD, the difficulty often involves an intense and repeated need to obtain certainty.
Someone may monitor their emotional or physical reactions, compare reactions toward different people, review previous relationships or experiences, repeatedly research sexual orientation, or ask others for reassurance.
The more the person checks, the more ambiguous their internal experience can begin to feel.
Rather than trying to diagnose the difference from a webpage, treatment begins with careful assessment of the pattern, the function of the checking, and the distress or impairment it is causing.
Patterns That Can Keep the Doubt Going
These are patterns, not proof of a diagnosis. They can appear in OCD and also in other conditions, and not everyone experiences all of them. Assessment can help clarify what is happening for you.
- Checking your reactions to people to see what they might mean.
- Reviewing past experiences, relationships, or memories for evidence.
- Comparing your responses with how you believe you should respond.
- Seeking reassurance from others or from online material.
- Avoiding people, media, or situations that tend to trigger the doubt.
What Therapy Does — and Does Not — Do
ERP is not used to determine someone's orientation.
It is not conversion therapy, and the therapeutic goal is not to move a person toward any particular identity.
When OCD is present, ERP focuses on the compulsive search for certainty. This may mean learning to notice checking, reviewing, reassurance seeking, or self-testing and gradually responding differently.
ERP is an evidence-based psychological treatment for OCD.
Creating Space for Authenticity
The goal is for OCD to take up less space.
When the repetitive checking and analysis become less dominant, there can be more room to experience relationships, attraction, identity, and life without continually monitoring what each moment “means.”
